TLC Team offer a wide range of support services for young people with various needs to help them live independently.

​

We do this through a combination of specialised housing management services, one-to-one support, signposting, drop-ins, group sessions and access to universal services.

Our Services

Solo Placements

There is no single definition or specific criteria for a solo placement with TLC Team.

 

Each placement is individual, so when considering if this type of provision is appropriate, be rest assured that we have the necessary skills, experience and ability to provide placements for young people experiencing multiple complex needs.

  • Skills and experiences of staff are matched with the young persons needs.

  • Established routines are embedded

  • Staff teams comprise of consistent cohorts; we find this really positive for the young person.

  • Engaging activities are incorporated.

  • Young people develop trusting relationships with a dedicated staff team.

  • Ongoing training provided for staff development in support of the placement and any identified needs.

  • Risks are identified and mitigated with identifiable strategies.

Group Living

We provide supported group living placements for young people to learn how to live independently.

 

Homes are shared with other young people aged 16-18 who have had similar experiences and who are also working towards their journey into adulthood and independence.​

  • Young people receive a bespoke plan supported by staff to achieve their outcomes

  • Staff are on site 24 hours a day, 365 days a year

  • Young people have the opportunity to learn and develop new skills and work towards becoming independent

  • Regular key-working sessions

  • Monthly progress reports

  • Assistance enrolling onto training/education courses, applying for work and emotional support as and when required

  • Working in collaboration with professionals involved in young people lives

  • Support staff provide assistance with daily living skills including budgeting, cooking, support with appointments

Supported Living

We offer young people who are close to independence the opportunity to live alone in their own fully furnished flat/apartment with support staff providing assistance for between 5 - 10 hours a week. Young people will receive support with their utility bills for the first 2 - 3 months by means of a subsidy and then take over full responsibility. Staff can provide assistance for pre-arranged appointments and can offer support over the phone and in person in an the event of an emergency.

Floating Support

For young people who are more or less independent and have managed to secure their own property, TLC Team can provide a floating support service to assist with managing situations and or situations requiring specialised support.
